North Putnam Family Healthcare Reopens!

Please welcome Johann (John) Farley, MD, MS!

Dr. Farley is board certified in Family Practice and will be caring for patients in the North Putnam Family Healthcare office beginning Wednesday, February 3rd.

Clinic hours will be Wednesday, Thursday and Friday from 9 a.m. to 5 p.m. weekly. For an appointment, please call 765-522-2556.

Temporary Visitation Restriction

For the protection of patients, visitors and staff, the Putnam County Hospital has chosen to implement an age restriction for visitors during this year's flu season.

It is recommended parents do not bring children aged 16 years or younger to visit the facility. This temporary age-based restriction is effective immediately.The decision was made in preparation of the H1N1 and influenza season.
