      <description>Putnam County Hospital is looking for an experienced Nurse Practitioner to work in our Family Medicine of Cloverdale physician office.


 
Tasks and Responsibilities include:
•Assess the health status, illness conditions, response to illness and health risks of individuals, families, and groups, employing the skills of taking histories, conducting physical examinations, and using laboratory and other diagnostic data. 

•Diagnose the actual or potential health problem and need, based upon analysis of the data collected. 

•Plan therapeutic interventions jointly with the patient. The goal is to develop the problem solving and self-care abilities of the patient to the greatest possible extent. The diagnosis gives direction to the plan of care, although the plan may include actions needed to further delineate (or diagnose) the problem. 

•Evaluate with the patient-and when indicated, with the collaborating health care provider, team, and/or consulting specialist-the effectiveness, comprehensiveness, and continuity of the intervention through assessment, modification of the plan, if necessary, and documentation of health care outcomes. 


Requirements 

Qualified candidates must have a Masters of Nursing Science (MSN) from an NLN or CCNE accredited school of nursing with preparation as a FNP; currently licensed registered nurse in the state of Indiana; Board Certified; Nurse Practitioner authorization or eligible and 2 plus years of experience in a physician office setting preferred.
