The hospital’s business office is responsible for the filing of insurance claims and the collection of co-payments and deductibles. The filing of insurance claims is based on the information provided by the guarantor (the patient, if over 18 years of age, or guardian of a minor) and the insurance carrier at the time of service. Corrections or updates to information provided at the time of service are the responsibility of the guarantor.
Most insurance carriers enforce a filing limit on providers (hospitals, doctor offices, etc.). These limits range from 60 to 180 days based on the carrier’s contract. If updated information is provided past your carrier's filing limit, then the claim will be filed as a courtesy. However, if it is past the filing limit, denials will not be honored.
The office is open from 8:00 a.m. to 5:00 p.m. Monday through Friday, excluding major holidays. For questions regarding a payment on an account or an insurance claim, please call 765-658-2719.
For Financial Assistance, please contact our our Business Office at 765-658-2719 . If you are in need of assistance for your recent hospital care, we may be able to help. Please bring the following items, along with the completed Charity Care Form (available in the Business Office), with you to your appointment:
- A copy of your most recent Federal tax form(s) with all schedules, including W-2s
- A copy of your most recent three (3) paycheck stubs
- A copy of your most recent three (3) bank statements for each account
- A list of your outstanding medical debts and monthly pharmacy costs
- The name and telephone number for your Medicaid caseworker, if applicable
