Did you know that 23.6 million children and adults in the United States, or 7.8% of the population, are living with diabetes? Alarmingly, 1/3 of those individuals are unaware that they have the disease! Putnam County Hospital recognizes the importance to support our communities with programs to help identify and support those with diabetes and to educate people regarding best health choices to assist in their prevention of diabetes.
The Diabetes Self Management Education program of Putnam County Hospital is designed to provide the community with a resource for education and support to allow individuals and families with diabetes to live to their greatest level of individual independence and wellness.
The DSME program includes team guidance from a Registered Nurse, Registered Dietitian and Rehabilitation Services staff. A physician order is required for program services which include, but are not limited to, self management education, medication/insulin instruction, nutrition counseling, diabetes foot screening, continuous glucose monitoring and insulin pump support.
Team members are also available to participate in community wellness screening and programming.

Do you have a child living with Diabetes?
A Youth and Family Support Group is being offered. Click Here for more information. First meeting will be held March 19 at 6:00 p.m. at the hospital.
Diabetes Self Management Education Group Classes
The series includes two sessions: Diabetes 101 and Diabetes 201.
Diabetes 101 is led by a Registered Nurse who will discuss topics including, but not limited to, "What is Diabetes?", potential complications, high and low glucose symptoms, glucose testing and medications.
Diabetes 201 is led by a Registered Dietitian who will provide a foundation for proper balanced nutrition to manage your diabetes. Topics will include label reading, portion control, carbohydrate counting and dining out strategies.

Putnam County Hospital has two Registered Dieticians on staff who offer scientifically based, objective nutritional information. This information is not subject to current fads and wild claims. They provide the personal qualities and experience you can trust to help you achieve your goals in weight loss or any other dietary need.
