Please understand most Insurance Companies have filing Limits so there is an Importance in contacting our Office so that we may assistance in a timely manner, to avoid 100% patient responsibility.

Financial Assistance Program
Part of being your indispensable healthcare partner means offering a patient-friendly Financial Assistance Program. Opportunities for assistance are available to all qualifying patients – regardless of whether you have insurance. The program includes all of our medical departments and facilities, from our ER and inpatient care to our outpatient services.

Please bring the following items along with the Completed Financial Assistance Application with you to your Appointment.
- A copy of your most recent Federal Tax Form(s) with all schedules
- A copy of your most recent Three (3) paycheck stubs
- A copy of your most recent bank statements for each account
