North Putnam Family Healthcare is a primary care office that offers a range of healthcare services to all ages. We offer lab services, wellness physicals, and sports physicals. We offer personalized healthcare close to home. Medication needs are conveniently located in the same building at Crossroads Care Pharmacy. We are also excited to announce that we are providing short-term in office therapy as well as behavior health medication management.


Disclaimer: In order to utilize our behavioral health services, a person must be an established patient with our practice and it requires an internal referral based on need.

I am a board certified Family Nurse practitioner since 2022 in the primary care setting with a  recent wound care certification. I have been a Registered Nurse since 2009. I obtained my Bachelor and Master of Science in Nursing at Indiana Wesleyan University. I work currently with the geriatric population in skilled nursing facilities. Also, I work with hospice companies and a wound specialty team.


I am passionate about providing comprehensive, compassionate care for our community. My special interest include geriatric, hospice/palliative care and wound management. I believe in in building strong relationships with my patients and working collaboratively with the many aspects of the skilled nursing community.


Outside of my practice, I enjoy spending time with my husband and two children. I spend my time with family and friends. My hobbies include reading and walking for exercise. 

Kelsey is a Family Nurse Practitioner dedicated to compassionate, patient-centered care. Kelsey graduated from Purdue University Global, bringing eight years of experience in cardiac care to her advanced practice role. Kelsey is committed to improving patient outcomes through collaborative care and attention to detail.


Outside of her professional work, Kelsey enjoys traveling and exploring new cultures, which has broadened her perspective and strengthened her ability to connect with patients from diverse backgrounds.

Jasmine Johnson, PA-C is a board-certified Physician Assistant specializing in Family Medicine. She graduated from Butler University in 2026 with her Master of Physician Assistant Studies, where she spent time building her clinical skills in primary care and preventative health. She attended Purdue University where she received her B.S. in Brain and Behavioral Sciences.


Jasmine is passionate about creating meaningful patient relationships with not only her patients, but all patients she may interact with. She understands that health is a team sport, and aims to equip patients with the necessary education and resources to succeed and reach goals.
