North Putnam Family Healthcare is a primary care office that offers a range of healthcare services to all ages. We offer lab services, wellness physicals, sports physicals, and radiology services. We offer personalized healthcare close to home. Medication needs are conveniently located in the same building at Crossroads Care Pharmacy. We are also excited to announce that we are providing short-term in office therapy as well as behavior health medication management.


Disclaimer: In order to utilize our behavioral health services, a person must be an established patient with our practice and it requires an internal referral based on need.

Lori ArmoldLori Armold earned her ASN in 2012 and BSN in 2014 from the University of South Dakota. She has worked as a Registered Nurse in the Coronary ICU at Rapid City Regional Hospital in South Dakota, in the Emergency Department at IU West Hospital in Avon, and in the ICU at Putnam County Hospital. While pursuing her MSN at Indiana State University, she completed clinical rotations at North Putnam Family Healthcare and was happy to accept a position there upon completion of the Family Nurse Practitioner program.


Catherine is a PA or physician assistant who graduated from Butler University with a B.A. in psychology and a Master’s in Physician Assistant Studies. Between graduating from her undergraduate studies and returning to Butler for PA school, she worked as a traveling consultant for her sorority, Alpha Chi Omega and as a psychometrist at St. Vincent Hospital, conducting inpatient and outpatient neuropsychology testing. She has been working for Putnam County Hospital since she graduated as a PA in 2019. Originally from Wisconsin, Catherine is a proud cheesehead and Packers fan. In her free time, she enjoys traveling, baking, exercising, volunteering, and spending time with family.
