Don’t have a Patient Portal account?

The patient portal allows easy access to pay your bill online or view your medical records.

To sign up for the Patient Portal, please visit the Medical Records Department, located at the Ground Level (basement) of Putnam County Hospital. Face-to-face verification of the person registering for this account is required.

Registration for the Patient Portal also takes place during the registration process at the hospital. The patient is simply asked for their personal email address, and instructions on how to create the account is sent via email
