Putnam County Hospital Physician Practices, including Family Medicine of Cloverdale, North Putnam Family Healthcare, Putnam Pediatrics and Internal Medicine, and Putnam Surgical Specialties, are now offering the Physician Practices Patient Portal.

The portal will allow patients to better manage their health by allowing electronic access to health records, request prescription refills, schedule appointments, receive appointment reminders via email and communicate with their provider through secure messaging.

Patients can now sign up for the Physician Practices Patient Portal during the registration process at any of the PCH Physician Practices locations.

All PCH Physician Practices locations are accepting new patients.
