Putnam County Hospital Physician Practices will offer back-to-school sports physicals during special hours beginning April 1 and continuing through the summer for area student athletes.

Physicals are given by appointment, at any of the following three physician
practices: Family Medicine of Cloverdale, North Putnam Family Healthcare and Putnam Pediatrics and Internal Medicine. The fee is $25 and is not billed to insurance. Please bring a list of all current medication with you.  The goal is to make sure that every student is physically able to play the sport for which he or she is pursuing.

Students should bring a copy of the IHSAA physical form to their appointment.  This form can be picked up at their school or can be downloaded at  https://www.ihsaa.org/Schools/Forms.
